I, the undersigned, authorize Countryside Companion Veterinary Services to photograph my pet(s) and use photos inthe reception area, on social media, and on our website. This release will remain in effect until you notify us in writing of any changes.

We will gladly prepare a written estimate of service fees if you desire. All fees are due at the time of services. We accept all major credit cards, check, cash, Care Credis, and Scratchpay. *

I hereby agree that all of the above information is correct to the best of my knowledge and I assume responsibility for the animal(s) listed.*

Appointment Arrival: Please arrive 5 minutes prior to your scheduled appointment time. Walk-ins and emergencies will be accommodated on a first come first serve basis.*

Behavior: We will give every patient a chance and we work very hard to keep them comfortable during their visit. If the patient becomes too scared or nervous to allow an exam that is safe and comfortable for us/them, we will attempt to see them with medication on board. If we give medication and are unsuccessful, the next step is sedation. If we are unable to safely sedate the patient, we will not be able to provide any further services to them. *

Late Arrival/Cancellation: If you are more than 5 minutes late, we reserve the right to reschedule your appointment for a later date/time. If we can accommodate you, you may be treated as a walk-in or asked to drop off. Please call us if you believe you are going to be late.*

Missed Appointment: No call/no shows impact our services. You will be required to place a $50 deposit prior to future appointments.*

Patient Arrival: For your protection as well as that of others and their pets, all dogs must be on a leash and all cats must be in a carrier. All pets must be properly controlled while in the waiting area and the exam room. If you have a retractable leash, you will be asked to exchange it for one of our slip leads.

If your cat is afraid of the carrier, you can help them acclimate to it in the days prior to your appointment by keeping it out and open near their food.*

Medication Refill Requests: Please allow our staff 24-48 hours to fulfill any medication requests. If it is a special-order medication, please call us 5-7 days prior to you running out. Requests can be made in person during an appointment, text/email, or over the phone. We also accept online pharmacy requests. Ask us about MyVetStore today! 

After-Hours Emergency: We are a small, independently owned veterinary office and operate only during scheduled hours. If your pet is having an emergency outside of these hours, please go to the closest 24 hour emergency clinic. 

Curbside: If you come in for an appointment and you are ill, we will proceed with your appointment as curbside. We are a very small practice and if 1 staff member becomes sick, it greatly impacts our clinic.*

All employees at Hutto Veterinary Clinic treat clients with respect and we expect the same treatment in return. If any of our staff feels threatened or is the recipient of disrespect or abusive language/actions from a client, we will immediately terminate our relationship. We have a zero-tolerance policy for a negative environment.  *
